Testosterone Therapy Treatment at ALPHA

What to bring to your appointment when transferring treatment

For all patients who are interested in transferring for Testosterone Therapy, we ask that you refrain from taking an injection from your legacy provider for at least 5 days before your appointment at ALPHA. This will enable our in-house blood test to operate accurately, and also for any external labs panels required to be accurate.

If possible, please bring the following items to your initial consultation:

  1. Copy of your most recent lab work (within 3 months)
If you do not have lab work taken within 3 months, we will need to draw and send external labs

  2. Details of any Prescriptions that you currently have related to treatment that require transfer. Bring cell phone pictures if possible.

  3. Details of your current Weight Loss or Testosterone dosage information, so that our providers may take this into consideration when defining your ongoing treatment plan. For any at-home medications, please include photographs of these clearly showing the label and that the medication bears your name as the patient.

If you’re ready to make an appointment, click the link to book or call/text us at 817-410-7007. If booking online, select your preferred service, and indicate that you are a transferring client.
